Support Planning

 

This is the time when the opportunity to say exactly what you need and to exercise your right to choose from the many options available to you that will improve your quality of life.

 

Having a well considered support plan completed, and purchasing from a quality provider will save you time and money and increase the chances of achieving your goals with the help of experienced staff.

 

Finances are limited and we understand the need to get more for less so we make every effort to include extras that will be useful to you at no extra cost.

 

SACMHA can help you to write your support plan detailing how you would like your life to look and what support you want to purchase with your personal budget. We will work closely with your Care Coordinator and an­yone else you want to be involved, to do all we can to meet your needs so that you achieve the outcome you want.

 

Location

Our trained workers can meet you at home, in the community or at SAC-MHA's Resource Centre for a one-off session or over several meetings un­til you are happy with your plan.

 

Date & Time

Hours are flexible and negotiable.

 

Cost

Will depending on the complexity of the support plan and the time needed to complete it.
